目的:观察香芍颗粒对围绝经期患者失眠认知行为疗法(CBT-I)效果的影响。 方法:选择麻醉与睡眠医学门诊收治的围绝经期失眠患者120例,年龄45~55岁。采用随机数字表法将患者分为两组:CBT-I组(C组)和香芍颗粒联合CBT-I组(X组),每组60例。C组接受为期8周的CBT-I治疗;X组在C组基础上加用香芍颗粒治疗,每次1袋,每日3次,连续口服8周。记录治疗前、治疗后4、8周的匹兹堡睡眠质量指数(PSQI)评分、Kupperman评分、汉密尔顿焦虑量表(HAMA)和汉密尔顿抑郁量表(HAMD)评分。使用额贴式睡眠记录仪(UMindSleep)监测治疗前、治疗后2、4、8周的睡眠数据。记录治疗期间脱落情况及不良反应发生情况。 结果:与治疗前比较,治疗后4、8周两组PSQI评分、Kupperman评分、HAMA评分和HAMD评分均明显降低(P<0.05),治疗后2、4、8周两组睡眠总时间明显延长,睡眠效率、深睡眠和快速眼动睡眠占比明显增加,睡眠潜伏期明显缩短,浅睡眠占比明显降低(P<0.05)。与C组比较,X组治疗后4、8周的PSQI评分、Kupperman评分、HAMA评分和HAMD评分明显降低(P<0.05),治疗后2、4、8周睡眠总时间明显延长,睡眠效率、深睡眠和快速眼动睡眠占比明显增加,睡眠潜伏期明显缩短,浅睡眠占比明显降低(P<0.05),治疗期间的脱落率明显降低(P<0.05)。两组胃肠道不适、治疗相关疲劳和短期焦虑加重的发生率差异无统计学意义。 结论:香芍颗粒可改善围绝经期患者CBT-I期间的睡眠质量,减轻焦虑抑郁情绪,降低治疗期间的脱落率。 |

Objective: To observe the effect of Xiangshao granules on the efficacy of cognitive behavioral therapy for insomnia (CBT-I) in perimenopausal patients. Methods: A total of 120 patients with perimenopausal insomnia in the outpatient clinic of anesthesia and sleep medicine, aged 45-55 years were enrolled. The patients were divided into two groups by random number table method: CBT-I group (group C) and Xiangshao granules combined with CBT-I group (group X), 60 patients in each group. Group C received 8 weeks of CBT-I treatment. Group X were treated with Xiangshao granules on the basis of group C, 1 bag each time, 3 times per day, for 8 consecutive weeks. The Pittsburgh sleep quality index (PSQI) scores, Kupperman scores, Hamilton anxiety rating scale (HAMA) and Hamilton depression rating scale (HAMD) scores were recorded before treatment, 4, and 8 weeks after treatment. The sleep data were detected by forehead patch sleep recorder (UMindSleep) before treatment, 2, 4, and 8 weeks after treatment. The dropout rate and adverse reactions during treatment were recorded in the two groups. Results: Compared with before treatment, the PSQI scores, Kupperman scores, HAMA scores and HAMD scores in the two groups were decreased significantly 4, and 8 weeks after treatment (P < 0.05), the total sleep time was significantly prolonged, sleep efficiency, deep sleep and rapid eye movement sleep ratio in the two groups were significantly increased 2, 4, and 8 weeks after treatment, and the sleep latency was significantly shortened, and light sleep ratio was significantly decreased (P < 0.05). Compared with group C, the PSQI scores, Kupperman scores, HAMA scores and HAMD scores in group X were significantly decreased 4, and 8 weeks after treatment (P < 0.05), and the total sleep time was significantly prolonged, and sleep efficiency, deep sleep and rapid eye movement sleep ratio in group X were significantly increased 2, 4, and 8 weeks after treatment, and the sleep latency was signficantly shortened, and light sleep ratio was significantly decreased (P < 0.05), and the dropout rate in group X during treatment was significantly lower (P < 0.05). There was no significant difference in the incidence of gastrointestinal discomfort, treatment-related fatigue, and exacerbation of short-term anxiety between the two groups. Conclusion: Xiangshao granules can improve the efficacy of CBT-I perimenopausal patients' sleep quality, reduce anxiety and depression, and reduce the shedding rate during treatment. |
